Information from Lift Dental PLLC dba Artistic Smiles about a data security incident and steps you can take to help protect your personal information.

We are writing to inform you of a data security incident that involved some of your personal information. At Lift Dental PLLC dba Artistic Smiles (“Artistic Smiles”), we take the privacy and security of your personal information very seriously. This is why we are offering information about steps you can take to help protect your personal information.

What Happened?

We recently discovered unauthorized access to our network occurred between June 14, 2026 and June 21, 2026. Upon discovering the incident, we immediately took steps to secure our systems and launched a thorough investigation to determine the nature and scope of the event.

What Information Was Involved?

Based on our investigation, the personal information that may have been involved includes your name, address, date of birth, and dental/medical treatment information.

What Are We Doing?

Upon discovery of the incident, Artistic Smiles immediately launched an internal investigation and engaged cybersecurity experts to assist us in determining what occurred and what information may have been affected. We have reported this matter to the appropriate authorities, including law enforcement. We are also implementing additional security measures and enhanced safeguards to reduce the likelihood of a similar event occurring in the future.

What You Can Do

We encourage you to remain vigilant by reviewing your account statements and monitoring your credit reports for any unauthorized activity. Please refer to the Additional Resources section below for steps you can take to protect yourself, including information about placing fraud alerts and security freezes on your credit file.

Contact Information for the Three Nationwide Credit Reporting Agencies

Free Credit Report

Under federal law, you are entitled to one free copy of your credit report every 12 months from each of the three nationwide credit reporting agencies. You can order reports online at AnnualCreditReport.com, by calling 1-877-322-8228, or by completing the Annual Credit Report Request Form available from the Federal Trade Commission and mailing it to Annual Credit Report Request Service, P.O. Box 105281, Atlanta, GA 30348-5281.

Colorado residents may obtain additional copies of their credit reports from each credit reporting agency as provided by state law. We encourage you to review your credit reports carefully and report any inaccurate or suspicious activity.

Fraud Alerts

You may place a fraud alert on your credit file at no cost. A fraud alert tells creditors to take additional steps to verify your identity before extending credit. Contact any one of the three nationwide credit reporting agencies listed above; the agency you contact is required to notify the other two.

Security Freeze

You may also place a security freeze on your credit report at no cost. A security freeze prevents credit reporting agencies from releasing your credit report without your express authorization, which may delay or prevent the approval of new credit, loans, or services in your name.

To request a freeze, contact each credit reporting agency separately. The agency may ask for your full name, Social Security number, date of birth, current and previous addresses, a copy of a government-issued identification card, and a recent utility bill or bank statement. Please confirm requirements directly with each agency before sending personal information.
